An intolerance to salicylates found in plants, medications, and manufactured products.

The salts and esters of salicylic acid are known as salicylates. They occur naturally in plants and act as a natural immune hormone and preservative that protects against diseases, insects, fungi, and harmful bacteria. They can also be found in many medications, perfumes, solvents, and preservatives.

People with salicylate intolerance are unable to consume a normal amount of salicylate without adverse effects. Because of the potential effects, diagnosis should only be conducted by a qualified medical professional. Salicylate sensitivity differs from salicylism, which is an overdose of salicylates like aspirin.

Symptoms can be mild or can be life-threatening. The classical symptoms are in the respiratory tract: rhinosinusitis, nasal and sinus polyps, or bronchial asthma. When polyposis and asthma occur together with analgesic intolerance, this is known as the triad. Up to 2.5% of the European population is affected and about 10% of intrinsic asthmatics. The rate of chronic sinusitis with nasal polyps is even higher. The skin and the gastrointestinal tract may also be affected by salicylate intolerance. Sufficient consumption of salicylates may lead to chronic conditions such as utricaria.

The most reliable form of prevention and therapy is to avoid salicylates. Complete avoidance or elimination may not be practical nor suitable for all sufferers. Potential alternative solutions include medication and desensitization.

  • Asthma (Rhinosinusitis) – Salicylate sensitivity is more likely to occur in people who have moderate to severe asthma or chronic rhinosinusitis. The problem becomes more common as people age and as their asthma gets worse.
